When were you saved?
Briefly share your testimony:
Have you been baptized in water?
Have you received the baptism in the Holy Spirit with the initial evidence of speaking in tongues?
If you have not received the baptism in the Holy Spirit, are you actively seeking this gift?
Do you accept the Bible as the all-sufficient rule of faith and conduct?
Are you in agreement with the 16 Fundamental Truths of the Assemblies of God? (See below for reference).
Will you support the work of this church with your prayers, presence, service, tithes and offerings?
Is there anything else you'd like to share?
THE 16 FUNDAMENTAL TRUTHS OF THE ASSEMBLIES OF GOD
The Scriptures Inspired – The Bible is the inspired and only infallible, authoritative Word of God.
The One True God – There is one true God, revealed as Father, Son, and Holy Spirit (the Trinity).
The Deity of the Lord Jesus Christ – Jesus is the eternal Son of God, fully divine and fully human.
The Fall of Man – Man was created good but fell by voluntary transgression, incurring physical and spiritual death.
The Salvation of Man – Man’s only hope of redemption is through the shed blood of Jesus Christ.
The Ordinances of the Church – Water baptism by immersion and Holy Communion (the Lord’s Supper).
The Baptism in the Holy Spirit – All believers are entitled to and should ardently expect and seek the baptism in the Holy Spirit.
The Initial Physical Evidence of the Baptism in the Holy Spirit – Speaking in tongues, as experienced on the Day of Pentecost.
Sanctification – An act of separation from evil, and dedication unto God; a progressive lifelong work of grace.
The Church and Its Mission – The Church is the Body of Christ and has a threefold mission: evangelize, worship, and disciple.
The Ministry – A divinely called and scripturally ordained ministry is provided by God to lead the Church.
Divine Healing – Deliverance from sickness is provided in the atonement and is a privilege of all believers.
The Blessed Hope – The rapture of the Church at Christ’s coming.
The Millennial Reign of Christ – The second coming of Christ, including the rapture, followed by His thousand-year reign on earth.
The Final Judgment – There will be a final judgment in which the wicked dead will be raised and judged.
The New Heavens and the New Earth – “We, according to His promise, look for new heavens and a new earth wherein dwells righteousness.”
